维生素D水平与富血小板血浆对老年患者膝关节骨性关节炎治疗效果相关性分析
Correlation analysis of vitamin D levels and the treatment effect of platelet-rich plasma therapy in patients with knee osteoarthritis
目的:探讨维生素D水平与富血小板血浆(PRP)对老年患者膝关节骨性关节炎治疗效果的相关性分析及对衰弱程度的影响。方法:选取2022年7月至2022年12月就诊于长治医学院附属长治市人民医院疼痛科行富血小板血浆治疗的膝关节骨性关节炎患者109例。所有患者于治疗前检测血清维生素D含量,同时评估西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、评测步行速度、进行计时起立-行走测试。并于治疗后3、6个月再行WOMAC评测、评测步行速度、进行计时起立-行走测试、评估治疗效果及衰弱程度。采用相关性分析评价维生素D水平与PRP对老年患者膝关节骨性关节炎治疗效果之间的相关性。结果:入组患者中68例患者(62.4%)为维生素D缺乏患者,41例患者(37.6%)为未缺乏患者。维生素D缺乏患者在治疗前的WOMAC评分明显高于维生素D非缺乏患者( P<0.05),同时,维生素D非缺乏患者治疗后3、6个月与治疗前评分差值也高于维生素D缺乏患者,差异有统计学意义( P<0.05)。术后6个月维生素D非缺乏患者较缺乏患者步行速度加快( P<0.05),计时起立-行走时间缩短( P<0.05),但两者在术后3个月未见明显差异( P>0.05)。血清维生素D水平与治疗后3、6个月WOMAC评分呈显著正相关( P<0.05)。 结论:血清维生素D水平与富血小板血浆对膝关节骨性关节炎治疗效果呈正相关。
更多Objective:To explore the correlation between vitamin D levels and the platelet-rich plasma (PRP) treatment outcomes in elderly patients with knee osteoarthritis.Methods:One hundred and nine patients with knee osteoarthritis received platelet rich plasma treatment in the Department of Painology in Changzhi People's Hospital affiliated to Changzhi Medical College from July 2022 to December 2022. The serum vitamin D levels, Western Ontario and McMaster University Osteoarthritis Index (WOMAC), walking speed and conducting timed stand up walk were evaluated before the treatment and at the 3, 6 months after the treatment. The treatment effect and degree of weakness were evaluated at 3 and 6 months after the treatment. Correlation analysis was performed between vitamin D levels and the effectiveness of PRP in the treatment.Results:Among the 109 patients, 68 patients (62.4%) were identified as vitamin D deficient (deficient group), while 41 patients (37.6%) were not deficient (non-deficient group). The vitamin D deficient group had significantly higher WOMAC scores before treatment compared to the non-deficient group ( P<0.05), but the non-deficient group showed greater improvement in WOMAC scores at 3 and 6 months after the treatment ( P<0.05). Six months after the treatment, the non-deficient group had faster gait speed ( P<0.05) and shorter timed up and go test duration ( P<0.05) than that in the deficient group; but no significant difference between the two groups at 3 months after treatment ( P>0.05). Serum vitamin D levels were significantly positively correlated with WOMAC scores at 3 months and 6 months after the treatment ( P<0.05). Conclusion:Serum vitamin D levels and the therapeutic effect of platelet rich plasma have a positive correlation in patients with knee osteoarthritis.
