目的:探讨消肿生肌汤口服配合五倍子汤加减熏洗在肛周脓肿手术后的应用效果。方法:队列研究。抽取2021年1月至2023年12月宝鸡市中心医院收治的86例肛周脓肿手术患者，按随机数字表法分为对照组和观察组，每组43例。对照组术后行高锰酸钾坐浴，观察组术后行消肿生肌汤口服配合五倍子汤加减熏洗治疗，两组均治疗3周。比较两组临床疗效、创面恢复情况(腐肉脱落时间、上皮组织新生时间及创面愈合时间)及治疗前后炎症因子[肿瘤坏死因子－α(TNF－α)、白细胞介素－6(IL－6)、C反应蛋白(CRP)]水平。结果:观察组治疗总有效率(95.35%，41/43)高于对照组(81.40%，35/43)， P<0.05。观察组腐肉脱落、上皮组织新生及创面愈合时间短于对照组( P<0.05)。治疗前，两组TNF－α、IL－6、CRP水平比较差异未见统计学意义( P>0.05)；治疗后，观察组TNF－α、IL－6、CRP水平低于对照组( P<0.05)。 结论:消肿生肌汤口服配合五倍子汤加减熏洗可加快肛周脓肿手术患者创面愈合，减轻局部炎症损伤。

Objective:To investigate the effect of oral administration of Xiaozhong Shengji decoction combined with modified Chinese gallnut decoction fumigation and washing on patients with perianal abscess after surgery.Methods:A total of 86 patients with perianal abscess who underwent surgery in Baoji Central Hospital from January 2021 to December 2023 were selected for the cohort study. And they were divided into a control group and an observation group by random number table method, with 43 cases in each group. The control group received potassium permanganate sitz bath after surgery, while the observation group received oral administration of Xiaozhong Shengji decoction combined with modified Chinese gallnut decoction fumigation and washing; both groups were treated for 3 weeks after surgery. The clinical efficacy, wound recovery (time for carrion shedding, epithelial tissue regeneration, and wound healing), preoperative and postoperative levels of inflammatory factors, including tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and C-reactive protein (CRP), were compared between the two groups.Results:The total effective rate of the observation group (95.35%, 41/43) was higher than that of the control group (81.40%, 35/43), P<0.05. The observation group had shorter carrion shedding time, shorter epithelial tissue regeneration time and shorter wound healing time compared with the control group ( P<0.05). There was no significant difference in the levels of TNF-α, IL-6 and CRP between the two groups ( P>0.05). After treatment, the levels of TNF-α, IL-6 and CRP in the observation group were lower than those in the control group ( P<0.05). Conclusions:Combination of oral administration of Xiaozhong Shengji decoction and modified Chinese gallnut decoction fumigation and washing can accelerate wound healing, and reduce local inflammation of patients after perianal abscess surgery.
