Abstract: Objective To explore the application value of integrated extended care model in self-management ability control of elderly patients with type 2 diabetes mellitus(T2DM).Method A total of 200 elderly T2DM patients who visited the Fourth Affiliated Hospital of Xinjiang Medical University from February 2023 to February 2024 were se-lected as the study objects and divided into control group(conventional nursing mode)and observation group(inte-grated extended care mode)by coin method,with 100 cases in each group.Glucose metabolism indexes,self-management ability and compliance were compared between the two groups.Results After nursing,the glucose me-tabolism level of the observation group was lower and the self-management ability score was higher,and the differ-ences were statistically significant(both P<0.05).Compared with the control group,the total nursing compliance rate in the observation group was higher,and the difference was statistically significant(P<0.05).Conclusion Elderly pa-tients with T2DM receive integrated extended care model,which can correct their glucose metabolism indexes and im-prove their self-management ability and compliance.
