Diagnostic significance of shear wave elastic imaging of rectus femoris muscle in diagnosis of COPD complicated with sarcopenia

Abstract: 目的:分析股直肌剪切波弹性成像(shear wave elastography, SWE)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)并发肌少症的诊断意义。方法:选择2022年12月至2023年12月我院收治的78例COPD患者,COPD并发肌少症32例为观察组,未并发肌少症46例为对照组,通过股直肌灰阶超声(grayscale ultrasonography, US)和SWE检查确定股直肌厚度(the thickness of the rectus femoris, RFthick)、股直肌横截面积(the cross-sectional area ofthe rectus femoris, RFcsa)和股直肌SWE平均弹性指数(SWEmean);通过Pearson相关系数分析股直肌数据与COPD临床特征。对比两组股直肌数据,绘制ROC曲线分析股直肌变量对COPD并发肌少症的预测。结果:Pearson相关系数分析显示:RFthick、RFcsa、SWEmean与FEV1、FEV1/FVC、6MWT呈正相关,与mMRC、CAT呈负相关;RFcsa与BMI值呈正相关,SWEmean与5STS、血清抵抗素、TNF-α、GDF15呈负相关(P<0.05)。观察组RFthick[(1.21±0.28)vs.(1.43±0.35)]cm、RFcsa[(3.82±0.38) vs. (4.09±0.41)]cm2和SWEmean[(9.62±2.15) vs. (11.45±3.08)]kPa低于对照组(P<0.05)。多因素Logistic回归分析显示:RFthick(OR=0.028)和SWEmean(OR=0.798)是COPD并发肌少症的影响因素(P<0.05)。ROC曲线结果显示:RFthick、SWEmean的AUC(95%CI)分别是0.735(0.624~0.827)、0.815(0.713~0.893)。结论:与灰阶US相比,SWE不受身高、体质量影响,体现COPD患者的骨骼肌功能;相较于RFthick,SWEmean对COPD并发肌少症诊断价值,可作为预测COPD并发肌少症具有意义。
