目的 采用UPLC-MS/MS同时测定小鼠血浆中15种胆汁酸的含量.方法 采用Shim-pack GIST C18色谱柱(100 mm×2.1 mm,2 μm),流动相A为水(含0.01%甲酸)、B为乙腈(含0.01%甲酸),梯度洗脱,流速0.3 mL·min-1,进样量5μL,柱温40 ℃,内标为胆酸-d4;多反应离子监测,采用负离子模式,电喷雾离子源,血浆样品经预处理后进样.结果 0.24~3.2 × 104 ng·mL-1 15种胆汁酸与其峰面积的比值呈良好的线性关系(r2≥ 0.9900);回收率为83%~110%,日内RSD为 0.68%~4.02%,日间 RSD为 1.53%~8.06%;提取回收率为 70.24%~94.25%;样品在-80 ℃下反复冻融和长期冻存后的稳定性良好.结论 所用方法操作简单、结果准确、重复性好,可用于测定小鼠血浆中胆汁酸的含量.

冻融周期中单囊胚与双囊胚移植的妊娠结局分析

目的:比较冻融周期中单囊胚与双囊胚移植的妊娠结局。方法:回顾性分析2012年1月至2016年12月于南宁市第二人民医院生殖医疗中心行冻融囊胚移植的3 675个周期。根据移植囊胚的数量和质量进行分组:(1)单囊胚组:包含单优组(移植1个优质囊胚)和单非优组(移植1个非优质囊胚);(2)双囊胚组:包括双优组(移植2个优质囊胚)、单优单非优组(移植1个优质囊胚+1个非优质囊胚)和双非优组(移植2个非优质囊胚)。进一步按患者年龄分层:<35岁、35~40岁和>40岁。比较单囊胚与双囊胚移植的着床率、临床妊娠率、多胎妊娠率、活产率、早产率、流产率的差异。结果:(1)单囊胚组患者年龄大于双囊胚组[分别为(33.2±4.7)和(31.5±4.3)岁],两组比较,差异有统计学意义( P<0.05);两组子宫内膜厚度、人工周期占比均无显著差异( P>0.05)。双囊胚组着床率、临床妊娠率、多胎妊娠率、早产率和活产率均显著高于单囊胚组( P<0.05),而流产率则显著低于单囊胚组( P<0.01)。(2)在<35岁患者中,双囊胚组多胎妊娠率和早产率显著高于单优组( P<0.01),双非优组的临床妊娠率、多胎妊娠率、活产率均显著高于单非优组( P<0.01)。(3)在35~40岁患者中,双优组临床妊娠率、多胎妊娠率、活产率均显著高于单优组( P<0.01);而单优单非优组和双非优组的临床妊娠率、活产率与单优组无显著差异( P>0.05),多胎妊娠率和早产率却显著增加( P<0.01)。双非优组的临床妊娠率、活产率、多胎妊娠率均显著高于单非优组( P<0.01)。(4)在>40岁患者中,与单优组比较,双优组并未显著提高临床妊娠率和活产率( P>0.05),单优单非优组的着床率和双非优组的临床妊娠率均显著下降( P<0.05)。双非优组与单非优组的着床率、临床妊娠率和活产率无显著差异( P>0.05)。 结论:无论患者年龄如何,若存在优质囊胚,应优先选择单优质囊胚移植;若无优质囊胚,也应考虑单囊胚移植,以期降低多胎妊娠风险,提高累积活产率,改善妊娠结局。

高效液相色谱串联质谱法测定大鼠血浆中伐地那非浓度的可行性研究

目的:建立一种基于高效液相色谱串联质谱技术的测定大鼠血浆中伐地那非浓度的方法,并评估其可行性。方法:采集正常Sprague-Dawley大鼠血浆样本。采用Phenomenex Synergi Polar-RP 80 A色谱柱(2.0 mm×50 mm,4 μm),柱温30 ℃,流动相A为0.1%甲酸水溶液,流动相B为0.1%甲酸乙腈溶液,流速为0.4 ml/min;质谱采用API 4000Qtrap三重四极杆质谱仪的正离子多反应监测模式监测,伐地那非和内标监测离子质荷比分别为489.3(Q1通道)/151.2(Q3通道)和466.4(Q1通道)/234.2(Q3通道);西沙必利为内标,检测大鼠血浆中添加的伐地那非浓度。然后评估该方法检测伐地那非浓度的专属性、线性及定量范围、精密度和准确度、基质效应、稳定性。结果:在选定的色谱和质谱条件下,伐地那非和内标的保留时间分别为2.62和2.80 min,峰形良好。该方法在0.2~200 ng/ml浓度范围内线性良好。该方法检测伐地那非的批内精密度(%CV)和准确度(%DEV)分别在1.5%~9.7%和-6.8%~6.6%之间。批间精密度和准确度分别在3.1%~8.4%和-3.7%~4.6%之间。均在要求的范围内。本样品处理方法,伐地那非的提取回收率在88.2%~104.6%之间,符合提取回收率的考察要求。该样品处理方法,伐地那非各质控浓度经内标归一化的基质效应因子(MF)分别为1.04、0.85、1.04,变异系数(%CV)在1.7%~10.7%之间,符合基质效应的考察要求。伐地那非的短期稳定性、长期稳定性、4次冻融循环稳定性的偏差都在±15%以内,变异系数均在5%以内。结论:本研究建立的高效液相色谱串联质谱法特异性好、准确性高,可用于检测大鼠血浆伐地那非浓度。

目的:建立一种基于高效液相色谱法(HPLC)的碘海醇检测方法,以测量小剂量和造影剂量注射条件下血液中碘海醇浓度。方法:该研究为方法学建立及评价。使用HPLC-UV系统(高效液相色谱仪+紫外检测器)建立方法,对该方法的线性、不精密度、回收率、检出限与定量限、携带污染进行性能评价,并对碘海醇在不同储存条件下的稳定性、血清和血浆碘海醇浓度的差异以及药物对方法的干扰进行初步评价。碘海醇在标本中的稳定性试验采用单样本 t检验,血清和血浆碘海醇浓度的比较采用配对样本比较的Wilcoxon符号秩和检验。 结果:碘海醇浓度在5~250 μg/ml( R2=0.999 9)和250~4 000 μg/ml( R2=0.999 8)范围内线性良好;在碘海醇浓度为20~3 000 μg/ml时,批内变异系数为1.63%~3.31%,批间变异系数为2.10%~4.09%,回收率为94.17%~106.13%%;检出限为1 μg/ml,定量限为5 μg/ml;在碘海醇浓度为4 000 μg/ml时,未检测到携带污染;室温下放置48 h,其浓度相对偏差为-5.55%~+5.58%,-80 ℃下反复冻融6次后,其浓度相对偏差为-1.28%~+6.68%;血清与血浆的测量结果未见统计学差异;缬沙坦等药物对此方法无明显干扰。 结论:成功建立了检测范围较宽的碘海醇HPLC检测方法,可稳定、准确的检测小剂量和造影剂量下碘海醇的血液浓度。

建立一种宏基因组二代测序 DNA流程检测BALF标本的性能确认方法

目的:建立一种宏基因组二代测序(mNGS)DNA流程检测BALF标本的性能确认方法。方法:以Hela细胞代表宿主细胞,向无菌生理盐水中加入不同浓度Hela细胞、7种病原体(肺炎链球菌、流感嗜血杆菌、肺炎克雷伯菌、白色念珠菌、新型隐球菌、烟曲霉和腺病毒)和干扰物质制备模拟BALF标本,同时收集临床BALF标本,以传统检测方法为参比方法,评价mNGS检测结果,确定mNGS的最低检出限、精密度、抗干扰能力、稳定性和准确度。结果:模拟标本中,肺炎链球菌、流感嗜血杆菌、肺炎克雷伯菌、白色念珠菌、新型隐球菌、烟曲霉和腺病毒7种病原体的最低检出限分别为150、262、102、67、96、83 CFU/ml和439 拷贝/ml。模拟阳性标本中所有病原体重复检测结果符合率均为100%。抗干扰试验结果显示,人源核酸浓度越高,mNGS检出病原体序列数越少。以大肠埃希菌和宋内志贺菌评价mNGS对近源物种分辨能力,结果显示,当宋内志贺菌浓度为4 000 CFU/ml时,该系统能稳定分辨大肠埃希菌和宋内志贺菌。稳定性试验结果显示,标本进行1、2、3次冻融或在4 ℃、-20 ℃、-80 ℃放置36 h后病原体序列数无明显变化。与传统检测方法相比,17份临床标本的初步准确度为82.4%(14/17)。2021年10月25日至2022年9月14日同济医院同时进行mNGS和传统方法检测的临床BALF标本的持续评估结果显示,mNGS相对细菌培养、真菌培养、分枝杆菌培养、结核分枝杆菌培养和常规PCR技术的准确度分别为67.5%(472/699)、81.5%(570/699)、92.3%(335/363)、96.4%(350/363)和86.8%(132/152)。与常规PCR技术比较,mNGS检测耶氏肺孢子菌、腺病毒、肺炎支原体的准确度分别为89.4%(84/94)、93.3%(56/60)和87.1%(61/70)。结论:通过制备模拟BALF标本以及以传统检测方法为参比方法,可以初步评价mNGS检测BALF标本的性能特征。

Nomogram to predict pregnancy outcomes of emergency oocyte freeze-thaw cycles

Background::Existing clinical prediction models for in vitro fertilization are based on the fresh oocyte cycle, and there is no prediction model to evaluate the probability of successful thawing of cryopreserved mature oocytes. This research aims to identify and study the characteristics of pre-oocyte-retrieval patients that can affect the pregnancy outcomes of emergency oocyte freeze-thaw cycles.Methods::Data were collected from the Reproductive Center, Peking University Third Hospital of China. Multivariable logistic regression model was used to derive the nomogram. Nomogram model performance was assessed by examining the discrimination and calibration in the development and validation cohorts. Discriminatory ability was assessed using the area under the receiver operating characteristic curve (AUC), and calibration was assessed using the Hosmer-Lemeshow goodness-of-fit test and calibration plots.Results::The predictors in the model of "no transferable embryo cycles" are female age (odds ratio [OR] = 1.099, 95% confidence interval [CI] = 1.003-1.205, P = 0.0440), duration of infertility (OR = 1.140, 95% CI = 1.018-1.276, P = 0.0240), basal follicle-stimulating hormone (FSH) level (OR = 1.205, 95% CI = 1.051-1.382, P = 0.0084), basal estradiol (E2) level (OR = 1.006, 95% CI = 1.001-1.010, P = 0.0120), and sperm from microdissection testicular sperm extraction (MESA) (OR = 7.741, 95% CI = 2.905-20.632, P < 0.0010). Upon assessing predictive ability, the AUC for the "no transferable embryo cycles" model was 0.799 (95% CI: 0.722-0.875, P < 0.0010). The Hosmer-Lemeshow test ( P = 0.7210) and calibration curve showed good calibration for the prediction of no transferable embryo cycles. The predictors in the cumulative live birth were the number of follicles on the day of human chorionic gonadotropin (hCG) administration (OR = 1.088, 95% CI = 1.030-1.149, P = 0.0020) and endometriosis (OR = 0.172, 95% CI = 0.035-0.853, P = 0.0310). The AUC for the "cumulative live birth" model was 0.724 (95% CI: 0.647-0.801, P < 0.0010). The Hosmer-Lemeshow test ( P = 0.5620) and calibration curve showed good calibration for the prediction of cumulative live birth. Conclusions::The predictors in the final multivariate logistic regression models found to be significantly associated with poor pregnancy outcomes were increasing female age, duration of infertility, high basal FSH and E2 level, endometriosis, sperm from MESA, and low number of follicles with a diameter >10 mm on the day of hCG administration.

ZNA探针在沙眼衣原体核酸定量检测中的研究

目的:探讨ZNA (ZIP Nucleic Acid)探针的PCR性能及其在沙眼衣原体(CT)核酸定量检测中的研究。方法:使用CT阳性质粒,比较偶联不同ZIP数量的ZNA探针的PCR扩增曲线差异;比较ZNA探针与29mer普通Taqman探针(DNA长探针)、20mer普通Taqman探针(DNA短探针)、MGB探针在PCR扩增曲线、反复冻融稳定性及低浓度质粒检出率的差异;使用CT阳性临床样本,比较ZNA探针与DNA长探针、DNA短探针、MGB探针扩增曲线差异及低浓度样本检出率的差异。结果:(1)偶联5个ZIP单位的ZNA探针Ct值和荧光值最优,与偶联1个ZIP的探针相比:Ct值提升1.34 (灵敏度提升2.37倍),荧光值提升30%。(2)偶联5个ZIP的ZNA探针的扩增效率是优选的普通Taqman探针和MGB探针的2.14~2.64倍,荧光值高17%~90%。(3)四组探针冻融稳定性结果显示,ZNA探针的稳定性最佳,低浓度(1×10 4拷贝/mL)的Ct值偏离最小(CV%= 1.4),优于DNA长探针、DNA短探针、MGB探针(CV%= 1.7~3.7)。(4)用35例CT临床样本比较四组探针(DNA长探针、DNA短探针、MGB探针、ZNA探针)的PCR扩增性能,相对于其他三组探针,ZNA探针的扩增灵敏度分别提升1.60倍、0.99倍和1.06倍,且Ct值一致性分析结果显示,ZNA探针对临床样本的检出性能提升更优。(5)使用低浓度质粒模板(分别为200、100、50和10拷贝/mL)比较四组探针的扩增灵敏度,ZNA探针的检出率均优于DNA长探针、DNA短探针、MGB探针;在最低浓度10拷贝/mL时,其他三组探针的检出率只有15%~20%,ZNA探针仍有30%。(6)在20例不同低浓度(分别为200、150、100和50拷贝/mL)临床样本中ZNA探针检出率最高,分别为100%、95%、90%和70%。 结论:偶联5个ZIP的ZNA探针的扩增效率、荧光值、冻融稳定性、临床样本的扩增性能及低浓度样本检出灵敏度都优于普通Taqman探针和MGB探针,ZNA探针可作为一种设计灵活和合成易得的新探针技术,在基因表达领域可进一步提升检测的灵敏度及低浓度样本的检出率。

目的:探讨>40岁女性冻融周期囊胚移植数目的选择策略,为降低双胎率、提高围产期临床结局提供参考。方法:回顾性分析2017年1月至2019年12月在广东省妇幼保健院生殖中心年龄>40岁、接受冻融囊胚移植的377个移植周期患者,根据囊胚移植数目分为单囊胚和双囊胚移植组。比较两组临床妊娠率、种植率、流产率、活产率、早产率、双胎率及单胎分娩率。结果:⑴两组取卵年龄、移植年龄、BMI、基础窦卵泡数(AFC)、基础卵泡刺激素(FSH)、抗苗勒氏管激素(AMH)、移植日子宫内膜厚度、获卵数、Gn启动剂量、Gn天数、Gn用量、优质胚胎数、D3可利用胚胎数比较差异均无统计学意义( P>0.05);⑵两组种植率、早期流产率、晚期流产率、活产率比较差异无统计学意义( P>0.05);⑶双囊胚组早产率(31.71%)明显高于单囊胚组(12.5%),但差异无统计学意义( P=0.083);⑷双囊胚组临床妊娠率、双胎妊娠率高于单囊胚组,差异有统计学意义( P<0.05);⑸双囊胚组单胎分娩率(75.61%)低于单囊胚组(95.83%),差异有统计学意义( P=0.036)。 结论:>40岁女性囊胚冻融移植,可选择单囊胚移植,保证活产率相当的情况下明显降低双胎率、提高单胎分娩率等围产期结局。

Supplementation with L-Carnitine Rescues Sperm Epigenetic Changes in Asthenospermic Male Semen with Altered Acetyl-L-Carnitine Levels

Objective::To investigate the relationship between the concentration of L-carnitine in semen and sperm parameters and investigate the epigenetic profile in sperm cell after L-carnitine usage.Methods::From February 2017 to February 2018, 46 semen samples from asthenospermic males and 41 semen samples from healthy donors were acquired. Motility parameters were assessed using computer-assisted sperm analysis (CASA, n = 78) and the DNA fragmentation index (DFI) was evaluated through flow cytometry ( n = 86), %DFI = % cells outside main population. Other oxidative stress markers, such as reactive oxygen species (ROS) levels ( n = 86) and the mitochondria DNA copy numbers, were detected ( n = 78). The concentration of L-carnitine and acetyl-L-carnitine was detected ( n = 82), and methylation was analyzed ( n = 30). After that, we collected 13 fresh semen samples from asthenospermic males and 23 fresh semen samples from healthy donors. These samples were used in a freeze-thaw model that was used to determine whether adding L-carnitine could change sperm progressive motility ( n = 23), apoptosis index ( n = 9), and methylation analysis ( n = 7). In total, we have done 13 asthenospermia samples for Western blot, and except for the poor Western result, we analyzed 6 samples for H3K9ac detection, 7 samples for H3K9m3 and H3K27m3 detection, and immunofluorescence ( n = 3). Finally, we had recruited 30 volunteers, and they were given oral administration of L-carnitine for 3 months and then collected semen samples at different time points for methylation analysis. Results::The concentration of acetyl-L-carnitine is negatively correlated with the %DFI value ( r2 = 0.1090; P = 0.0026), and the concentration of acetyl-L-carnitine is positively correlated with sperm forward motility ( r2 = 0.0543; P = 0.0458) and ROS ( r2 = 0.1854; P < 0.0001), and the acetyl-L-carnitine level is negatively correlated with %DFI in asthenospermia ( r2 = 0.1701; P = 0.0066), and the level of acetyl-L-carnitine in asthenospermic semen is significantly lower than the normal group ( P = 0.0419). In addition, this study indicates that adding L-carnitine significantly improved sperm motility ( P = 0.0325) and reduced sperm apoptosis ( P = 0.0032). Importantly, Western blotting ( P = 0.0429) and immunofluorescence staining results showed that the addition of L-carnitine reduced H3K9Me3 methylation level in sperm, respectively. Furthermore, semen samples from asthenospermic patients had reduced methylation levels in a specific region (16 thP= 0.0003; 17 thP= 0.0016) of the brain-derived neurotrophic factor ( BDNF) promoter. The 16 th methylation decreased with age ( r2 = 0.1564; P = 0.0306), and the 17 th methylation was decreased after treatment with L-carnitine for 28 days ( P = 0.0341). Conclusion::L-carnitine can reduce the %DFI and also affect the methylation of the histone modification marker in sperm as a possible epigenetic regulator.
